Why special emphasis on first 1000 Days of a child?
• 85% of under-five mortality occurs within the first year of birth
• 67% of infant deaths occur within the first 28 days
• 74% of all neonatal deaths occur within the first seven days of birth
• 37% of all child deaths in four weeks or 28 days of birth take place on day 1, or, within 24 hours of birth
• More than 90% of these deaths can be prevented if actions are taken on time
We have been systematically working in all the projects starting from Anti Natal Care (ANC), Post Natal Care of mother and children (PNC), growth monitoring of children, capacity building of pregnant and lactating mothers, ICDS workers, ANM, ASHA, regular networking and interface with Block and district health officials, Nutritional Rehabilitation centers. However, we have increasingly realized that the system takes its own time to respond and the system is yet to gear up fully to address the Post Natal Care of mother and children (PNC) which is very much critical for the neo-natal child.
Hence, while we will be implementing our core strategy of system strengthening and community empowerment, we intend to introduce the concept of CMC- Counseling and Monitoring centre as part of immediacy which will function towards:
• Curbing Anemia
• Preventing Teenage Pregnancy
• Care of Pregnant Women
• Institutional Delivery
• Colostrums feeding
• Exclusive breast feeding
• Complimentary feeding
• Child Under Nutrition
Curving Anemia: We will try to address the anemia through following way-
• Regular screening of anemia though Public Health System in each ANC
• Community awareness about signs and symptoms of anemia
• Iron Folic Acid supplementation program for Pregnant, lactating women and women in reproductive age group as per National Iron Plus Initiative Guidelines
• Generating awareness on side effects of IFA tablets consumption and how to overcome that (this will increase consumption of Iron Folic Acid)
• Ensuring Biweekly distribution of IFA Syrup throughout the period 6–60 months of age and de-worming for children 12 months and above through ANM / ASHA
• Ensuring Weekly distribution of Tablets of 45 mg elemental iron and 400 mcg of folic acid throughout the period 5–10 years of age and biannual de-worming through school and for out of school children through Anganwadi centre
Preventing teenage Pregnancy: We will try to address the anemia through following way-
• Delaying age of marriage through formation of adolescent groups and generating awareness among them about right age of marriage
• Regular meeting with newly married couples so as to delay pregnancy among them
• Generating awareness regarding different contraception methods available in health system
• Ensuring linkage of ASHA and ANM with newly married couples so as to serve their demand of contraception Delaying age of marriage through formation of adolescent groups and generating awareness among them about right age of marriage
Objectives of the Counseling and Monitoring centre
I. Objective of CMC- Care of Pregnant Women: we will do the care of pregnant women in the following way:
• Ensuring early registration of pregnancy at health system and Anganwadi
• Ensuring regular updating of information in MCP card provided to pregnant women
• Generating awareness on benefits of ANC check ups and tests to be done
• Linking pregnant women with different government schemes
• Ensuring immunization of all pregnant women and all three ANC check up of all pregnant women
II. Objective of CMC- Institutional delivery: We will ensure the institutional delivery in the following way:
• Ensuring regular ANC check up of every female in catchment area in hospital
• Community awareness on safe institutional delivery
• Linking of pregnant women with different schemes like Janani Suraksha Yojna , Pradhan Mantri Matritva Vandana Yojna etc.
• Identification of High risk pregnancy and ensuring their referral
• Ensuring delivery plan in advance for pregnant women
III. Objective of CMC- Colostrums Feeding: We will ensure colostrums feeding-
• Targeted counseling of all pregnant women in third trimester about benefits of colostrums feeding
• Clearing all the myths prevailing in community about colostrums feeding
• Reinforcing the importance of early initiation of breastfeeding and how to implement it successfully though duty bearers
• Developing posters / slogans for imparting knowledge on importance of colostrums feeding
IV. Objective of CMC- Exclusive Breast feeding: Provide information on:
• Benefits of exclusive breastfeeding
• Skills of positioning
• Attachment of infant
• expression of breast milk
V. Objective of CMC- Complimentary Feeding: We will ensure complimentary feeding through-
• Ensuring initiation of complementary feeding at completion of 6 months and awareness generation on possible hazard of starting or delaying initiation of complementary feeding
• Providing information to community on preparation of nutritious complementary foods
• Awareness on availability and nutritional value of locally grown food
• Imparting knowledge on how much to feed and what to feed (age appropriate feeding)
• Sensitizing community on responsive feeding
VI. Objective of CMC- Child Growth Monitoring: We will do child growth monitoring through-
Monitoring and recording periodic growth monitoring of every child
Awareness generation on how to identify growth faltering
Focused counseling of children identified as MAM and SAM
Prompt referral of any SAM child identified
